LA Spas Flow Switch 5EL-62048 Harwill Q-12DS-C2.  This particular version has a unique 3 position plug - other versions are available for less including the 6560-857 which is a tee with box end connectors on the end of the wires, or the 34-0221-k which has a tee with 2 position plug on the end of the wires.
