This is the QCA Spas Balboa 2 button control panel with cable and phone style connector included. 

This control panel also includes the QCA Spas graphic overlay already mounted to the panel.

This 2 button panel was used for a limited time on the Dream Star 2 person QCA Spas hot tub.

Dream Star also uses a 4 button control panel available here.
